Admission
Requirements
The following general requirements are
provided to assist you in understanding the basis of individual
acceptance of your application to the Traces of Tiger's retirement
lifestyle. These pre-requisites are necessary to ensure the highest
quality environment as well as compliance with the Rules and Standards
of the Georgia State Board of Health.
Social
Interview - A personal interview
with a representative of Traces of Tiger management (usually the
Administrator or Business Manager) will be conducted prior to final
application approval.
History - A Traces of Tiger
representative will make contact with the family or friends regarding
recent or past activities of daily living. Should management require
personal references, a written request will be provided.
Health
Mental Ability - Has the ability
to make prudent decisions regarding personal safety. Has the ability
to recognize and respond appropriately to emergency situations. Has no
mental illness or prior treatment for substance abuse.
Ambulation - Has the ability to
move independently throughout the building and evacuate the building
in case of an emergency.
Functional Ability - May require
assistance with personal hygiene, dressing, activities of daily
living, personal safety and medication normally prescribed for
self-administration.
Personal - Is free from any
contagious disease and has the ability to self-administer prescription
medication. Has no history of violent behavior and is not a danger to
residents or self.
Physician's Statement - A
statement or report from a licensed physician that includes a
diagnosis of present condition, any condition requiring special diets,
a listing of present medication and how often it should be
administered and a statement that the resident's personal care needs
can be met in a domiciliary facility by non-nursing personnel.
Other Eligibility
Criteria
1. Insulin-dependent diabetics capable
of measuring and self-administering the correct insulin dosage will be
eligible for admission. Residents who become unable to measure and
self-administer the correct insulin dosage will be required, after
proper notification, to vacate.
2. Individuals who are
permanently incontinent, as determined by a physician, will be
ineligible for admission. Residents who become permanently incontinent
after admission will be required, after proper notification, to
vacate.
3. Individuals with a colostomy
appliance who are capable of correctly caring for the colostomy and
its appliance will be eligible for admission. Residents with a
colostomy who become unable to care for the colostomy and its
appliance after admission will be required, after proper notification,
to vacate.
4. Individuals who require
indwelling or retention catheters will not be eligible for admission.
Residents who require indwelling or retention catheters after
admission will be required, after proper notification, to vacate.
5. Individual whose behavior is
indicative of mental illness and/or impairment and whose presence
would be a disruptive force or a threat to the residents or employees
will be ineligible for admission. Residents who, after admission,
evidence behavior which is indicative of mental illness and/or
impairment which causes trouble and is a disruptive force and/or is a
threat to the residents or employees, documentative evidence of such
being in the resident's file, will be required to vacate after proper
notification.
